About the Role

Clarvida is seeking a Health Navigator to coordinate with hospital systems and community partners to reduce barriers to care for individuals who may benefit from Tennessee Health Link (THL) services. In this role, you will build and maintain professional relationships with external agencies and facilities, engage hard-to-reach individuals, and educate potential members on how THL services can support their unique health and behavioral health needs.

The Health Navigator plays a key role in outreach, engagement, and connection to care, serving as a bridge between Clarvida, healthcare systems, and the communities we serve.

Responsibilities

  • Build and maintain professional relationships with hospital systems and community partners
  • Coordinate with external agencies and facilities to identify and engage potential clients
  • Conduct outreach to difficult-to-reach individuals and potential new THL members
  • Educate potential clients on Tennessee Health Link services and benefits
  • Reduce barriers to care by facilitating connections to appropriate services
  • Maintain accurate documentation of outreach and engagement activities
  • Collaborate with internal teams to support enrollment and continuity of care

Requirements

  • Bachelor’s degree in Social Work, Psychology, Mental Health, Child & Family Studies, Human Development, or a related field or Licensed Practical Nurse (LPN)
  • Experience working in the healthcare industry
  • Background in marketing, sales, or public relations preferred
  • Strong verbal and written communication skills
  • Ability to work independently and collaboratively with community partners
  • Ability to meet background check requirements

Compensation & Benefits

Hourly Pay Rate
$18.27/hour
