About the Role
We are looking for an Adult Case Manager to join our team. This position involves supporting clients with complex needs, including mental health, substance use, and other personal challenges. You will work directly with clients to create and implement individualized treatment plans, connect them with resources, and provide ongoing support to help them achieve their personal goals and live independently in the community.
Responsibilities
- Develop, implement, and monitor treatment plans for clients
- Provide advocacy and link clients to community resources
- Conduct assessments and coordinate with other service providers
- Facilitate client access to services and monitor progress
- Provide counseling and support services to help clients achieve their goals
- Attend team meetings and case conferences to ensure comprehensive care
Required Qualifications
- Bachelor's or Master's degree in a related field (e.g., Social Work, Psychology, Sociology)
- At least 1 year of experience in case management or behavioral health
- CPR/First Aid certification (Training provided if not current)
- Valid Arizona Driver’s License with no restrictions
- Eligible for fingerprint clearance approval
Preferred Qualifications
- Experience working with individuals in mental health or substance use recovery
- Strong communication and interpersonal skills
- Bi-lingual (English/Spanish)
