About the Role

Clarvida is seeking a Family Preservation Care Coordinator to provide intervention, case management, and resource linkage services to children and families in need. In this role, you will manage client cases, collaborate closely with treatment teams, and serve as an advocate for children and families facing significant challenges.

You will support family preservation efforts by helping clients access community resources, coordinating care plans, and contributing insight during treatment planning and evaluation. This position requires flexibility, strong collaboration skills, and a commitment to supporting families within community-based settings.

Responsibilities

  • Provide intervention and case management services to children and families
  • Manage assigned client cases and monitor progress toward treatment goals
  • Collaborate with treatment teams to develop and evaluate care plans
  • Serve as an advocate for children and families
  • Link clients to community organizations and resources that support stability and safety
  • Work with families, service providers, and community partners
  • Maintain accurate and timely documentation in accordance with program standards

Required Qualifications

  • Bachelor’s degree from an accredited four-year college or university
  • Experience working with children and/or adolescents in a therapeutic, community-based treatment environment
  • CPR and First Aid certification or willingness to obtain certification prior to working with clients
  • Ability to work flexible hours based on program needs
  • Ability to work across multiple locations
  • Ability to meet background check requirements

Compensation & Benefits

Hourly Pay Rate
$18.27/hour
