Care Coordination Services for Adults and Children
When several providers or systems are involved, it can be hard to know who to call or how everything fits together. Across Maine, Clarvida offers two care coordination services that help organize support around your goals: Behavioral Health Home (BHH) and High-Fidelity Wraparound (HFW).
Care coordination services help bring together the people, providers, and resources involved in a person’s care. Through regular communication, planning, and support, Care Coordinators help children, adults, and families navigate behavioral health, healthcare, educational services, and community programs more effectively.
Behavioral Health Home (BHH)
Behavioral Health Home (BHH) is a community-based care coordination service for eligible children and adults with MaineCare. It is not an actual place or residential home. Instead, a Care Coordinator helps bring together the behavioral health, medical, educational, and community supports that are important to you or your family under one coordinated plan.
How Behavioral Health Home Can Help
Your Care Coordinator will listen to what matters to you, help you set goals, connect you with helpful resources, and work with the people involved in your or your child’s care. Depending on your needs, this may include coordinating with doctors, therapists, schools, community programs, and trusted people in your support network. Your BHH team may also include a Nurse Care Manager, clinical and medical consultants, and peer support. Children and families may have access to family and youth peer support, while adults may work with a peer who has relevant lived experience.
Is Behavioral Health Home Right for You or Your Family?
Behavioral Health Home (BHH) may be a good fit for an adult with a qualifying mental health diagnosis or for a child or young adult under age 21 with a qualifying mental health, autism, or developmental diagnosis. The person must have MaineCare and meet the program’s eligibility requirements.
What You Can Expect
You will have at least one BHH in person visit each month, with additional contact based on your or your child’s needs and plan. Your Care Coordinator will stay connected throughout the month through visits, phone calls, telehealth, referrals, advocacy, and follow-up with providers or community resources. In-person visits may take place in your home or another appropriate community setting.
High Fidelity Wraparound (HFW)
High Fidelity Wraparound (HFW) is a more intensive care coordination service for eligible children and young adults with complex behavioral health needs who are involved with several services or systems. HFW helps young people remain safely at home and connected to their communities by bringing the important people in their lives together around one coordinated plan.
How High Fidelity Wraparound Can Help
A Wraparound Care Coordinator helps your family build a Child and Family Team made up of the people you choose, such as relatives, friends, school staff, providers, community members, and other formal or natural supports. Your family’s voice and choices guide the process. Together, the team creates one plan based on your family’s strengths, needs, culture, priorities, and hopes for the future. The team can help coordinate services, prepare for difficult moments, strengthen natural supports, and build solutions your family can continue using over time. Family and youth peer support may also be part of the team.
Is High Fidelity Wraparound Right for Your Family?
HFW may be a good fit for a child or young adult with complex behavioral health needs who is involved with multiple providers or systems and needs more intensive coordination. The child or young adult must have MaineCare, meet the clinical requirements, and be approved through Maine’s Single Assessment process.
What Your Family Can Expect
HFW includes at least one in person visit each week and at least one Child and Family Team meeting each month. Weekly contact may involve you, your child, or another person on the team, depending on your family’s plan and current needs. Between visits, the team continues coordinating supports, following up on commitments, preparing for changing needs, and reviewing progress.
