Home and Community-Based Treatment (HCT)

How HCT Can Help

Your HCT team will build on your family’s strengths and create a plan based on what has been difficult and what you would like to change. Support may include individual and family therapy, help with emotions and behaviors, caregiver coaching, practicing everyday skills, creating safety and coping plans, and working with schools or other providers. We will help your child and family learn practical ways to respond to difficult or intense moments with greater confidence and use those strategies in everyday life.

What Your Family Can Expect

Your family will work with a clinician and a Behavioral Health Professional (BHP). You are an important part of the team, and we will help you practice strategies that fit your family and can be used between visits. Families usually have an average of three HCT contacts each week, including at least one visit with the clinician. Visits may take place at home or in the community, and the exact schedule will depend on what your family needs.

Is HCT Right for Your Family?

HCT may be a good fit for a child or young adult under age 21 who has MaineCare and needs more support than regular outpatient therapy can provide. It is designed for young people whose mental health needs are making everyday life difficult at home, at school, or in the community. Because HCT works best when the family and team work together, parents or caregivers take part in the service. Maine uses a statewide process to decide whether a child meets the requirements for HCT.

Getting Started

The HCT referral process is managed statewide by Acentra Health. Families may make a referral themselves or, with their consent, ask a care coordinator, therapist, or another support person working with the child or family to help complete it. Clarvida can answer questions and help your family understand the next steps.
