Family Connections Program – Arizona

Family Connections services are provided to families referred by DCS by a Family Connections Consultant (FCC). The FCC provides community resources and parent education services to help parents safely care for their children. Family strengths are used to make positive behavioral changes in family functioning and to ensure the safety of children. The FCC provides services to referred families as outlined in the Arizona Family Connections manual to include: intake; outreach and engagement; concrete/emergency services; comprehensive family assessment, including the use of standardized assessment instruments; outcome-driven service plans with UBSMART goals; change focused intervention; evaluation of change; and service closure.

Family Connections is a multi-faceted community-based practice model that works with families in their homes, in the context of their neighborhoods, to help them meet the basic needs of their children, prevent child maltreatment, and/or eliminate danger. The FCC meets with the family one to two times per week for 5 months to work on identified treatment goals. There is an emergency phone number given to all participants during services that will ensure families can access assistance from the agency on nights and weekends also.

Nurturing Parenting Program – Arizona

Nurturing Parenting services are provided to families referred from DCS by a Nurturing Parenting Program Practitioner (NPP). The NPP provides parent education services to help parents safely care for their children. Family strengths are used to make positive behavioral changes in family functioning and to ensure the safety of children.

The Nurturing Parenting Program is a curriculum-based parenting skills education and coaching program to improve parenting in relation to: expectations of children, parental empathy towards children’s needs, non-violent discipline, parent-child family roles, and children’s power and independence. The NPP meets with the family 2 hours per week for 4 months to work on identified treatment goals. There is an emergency phone number given to all participants during services that will ensure families can access assistance from the agency on nights and weekends also.

Foster and Adoption Support Team (FAST) – Arizona

The FAST program was designed by Clarvida to support families and children that are involved in foster care, adoptive care or kinship care to increase their stability and to ultimately prevent disruption. These children must be enrolled in AHCCCS and Mercy Maricopa Integrated Care (MMIC), and they are or have been involved in the DCS/child welfare system. This program is designed to work directly with the entire family system, highlighting the strengths and needs to provide support over an intensive period of 60 days (on average). This program is intended to assist the primary caregivers in adapting to meet the needs of the children placed, prevent disruptions, and higher levels of care. FAST uses a trauma-informed perspective, as the belief is that every child that has been removed from a caregiver has potential trauma responses. A team is comprised of a Clinical Supervisor, a Family Support Specialist and a Family Resource Aide.

Therapeutic Foster Care – Arizona

Our Foster Care Programs provide therapeutic foster care services to a wide range of families and children. Clarvida of Arizona works with children, and families in these programs to ensure the best possible match and long-term success. Our programs also provide 24-hour support to ensure the best possible outcomes for our families. Clarvida of Arizona offers all of its Therapeutic Foster Care Services in all served regions across the state of Arizona.

Developmental Home Program – Arizona

The Developmental Home Program, established in 1998, serves individuals with developmental disabilities through a contract with the Arizona Division of Developmental Disabilities. This program utilizes licensed, family-home settings for residential placement that matches family strengths with members needs and encourages maximum family and community involvement. Strong agency-level support makes a difference in attitudes about individuals with disabilities.

Psychiatric Services – Arizona

Clarvida of Arizona offers psychiatric services, medication management, and assessment through its team of Psychiatrists and Nurse Practitioners. Services can be provided in-person or across the state via our tele-medicine technologies.

Family Therapy – Arizona

We provide systemic family therapy​ to address support for positive parenting and to strengthen family relationships and communication.
