CBRS Case Manager – Community-Based Services Professional

About the Role

Clarvida is seeking a CBRS (Community-Based Rehabilitation Services) Case Manager to provide home- and community-based support to clients ages 5 through adulthood who experience emotional disturbance, behavioral challenges, or severe and persistent mental health needs.

In this role, you will help clients build skills, access community resources, develop coping tools, and strengthen independence. You will maintain weekly contact with clients and provide documentation including progress notes, treatment plan updates, and discharge summaries. This position offers flexibility, hybrid work options, and strong growth opportunities within a national organization.

Responsibilities

  • Provide CBRS and case management services in clients’ homes and communities
  • Support skill building, emotional regulation, and coping strategies
  • Help clients access community resources and increase independence
  • Maintain documentation including daily notes, treatment plans, and discharge summaries
  • Meet with clients weekly to monitor progress and goals
  • Collaborate with supervisors, clinicians, and the treatment team
  • Utilize electronic health records for timely and accurate documentation

Required Qualifications

  • Bachelor’s degree required (Human Services strongly preferred: Social Work, Psychology, Sociology, Counseling, Family Relations, Nursing, OT/PT, Therapeutic Recreation, Speech Language Pathology, Behavioral Sciences, Art Therapy, etc.)
  • Ability to pass all required background checks, including Enhanced Fingerprinting with ID DHW
  • Valid driver’s license, reliable transportation, and proof of auto insurance
  • Experience with electronic health records (preferred)

Preferred Qualifications

  • Experience with serious mental illness or trauma-based populations
  • Familiarity with evidence-based modalities (CBT, DBT, EMDR, etc.)
  • Bilingual Spanish/English preferred
  • Experience in school or outreach settings

Compensation & Benefits

Hourly Pay Range
$18.00 – $21.00
